Navigating our healthcare system can be arduous for the best of us, and unmanageable for those with serious issues, or no support system. Our nurses will stay connected to patients and providers from admission to discharge, and follow through with all orders once patient arrives back to their home setting.

Navigating our healthcare system can be arduous for the best of us, and unmanageable for those with serious issues, or no support system. Our nurses will stay connected to patients and providers from admission to discharge, and follow through with all orders once patient arrives back to their home setting.
This service includes physical assessment and medication management on a weekly basis. Every aspect of our patients care will be addressed by a RN at this time, and coordination of care with providers and family will be performed.
$500/month after initial consultation
Initial Consultation includes: Physical assessment of patient. Safety assessment of their home. Retrieval of records from all treating healthcare providers. Medication management and set up, as well as placement of caregivers in the home should this be a requirement for safety. Set up will incorporate a delivering pharmacy, if requested. At this time, we will involve the patients Long Term Care Insurance policy if requested.
$500/includes a plan of care

With RNs certified in Hospice and Palliative Care, we can provide the most knowledgeable end of life care for patient and families. Platinum Healthcare also has a Certified Grief Counselor on staff for patient/families/friends.

Recovering at home after surgery can limit you in your daily activities. Whether you are recovering from major surgery, childbirth, or plastic surgery, our experienced nursing staff can assist you to a healthy and complete recovery.
RN $125/hour
Concierge Services

Post-Surgical Assistance
This service includes attending doctors appointments with patient, accompaniment on flights or other transportation where nursing care is needed, or additional home visits as requested by the patient or his/her family, and will be billed our hourly rate of $125/hour.
